If more than one Power of Attorney is requested for yourself or a family member, additional documents are $100 each. 

What is the Difference:

Healthcare Power of Attorney: a document in which you designate someone to be your representative in the event that you are unable to make or communicate decisions about any aspect of your healthcare

Durable Financial Power of Attorney: a document that can arrange for someone you designate to manage your finances should you become unable to make or communicate decisions for yourself.
